Your facility manages its patients, appointments, patient communication and medical records. MFMD provides the contracted medical-director oversight, reviews the agreed program and supports protocol maintenance. The treating prescriber evaluates each patient and makes the treatment decisions. Direct patient consultations and prescriptions require a separate clinical-care arrangement.

The $1,000 one-time package covers the agreed peptide protocols and shared clinical foundation: provider responsibilities, patient evaluation, consent, ordering, monitoring, documentation and escalation. We review suitable existing documents and adapt them to your facility. The written proposal identifies the accepted compounds, formulations and routes. The fee is for that agreed scope, with no fixed number of peptides promised.

The included treatments are documented after reviewing your proposed program. Send the compound name, formulation, route, intended use and proposed pharmacy. MFMD and the treating prescriber review each request before it enters the facility’s approved scope. A supplier catalog or a name in our educational resources does not establish acceptance.

FDA publishes substance-specific safety information for products proposed for compounding. Product status, evidence, formulation, route and lawful source must be reviewed at the time of acceptance. Off-label use of an approved drug is different from use of an unapproved substance. An advisory-committee recommendation is not FDA approval, and a research-use label is not authorization for patient treatment.

First, submit your facility, clinical team, proposed products and existing protocols. Next, confirm the accepted scope and written fee proposal. Then complete the agreed documents, sourcing review and staff readiness steps. Clinical services start on the agreed activation date after the physician confirms readiness. Later additions are reviewed before use; any additional protocol work is agreed in advance.

One-time peptide setup is $1,000. Ongoing MFMD Medical Wellness governance is $1,500/month for the contracted group, including one APP per facility; each additional APP is $500/month. Other contracted groups and optional services are quoted separately. This governance fee does not include direct patient visits or medications.

The clinical foundation is included in the selected protocol package, with no separate foundation charge. It covers the agreed operational manual, clinical responsibilities and delegation, evaluation and ordering workflow, consent and digital records, infection control, medication handling, emergency response and quality follow-up. Existing suitable documents are reviewed and reused. Each selected service adds its own protocol details.

Peptide protocol setup is $1,000 one time, including the shared clinical foundation. GLP-1 weight-management protocols are a separate $500 when requested, bringing both programs to $1,500. Medicines, laboratory tests, direct patient consultations and ongoing governance are separate. The proposal confirms the accepted program before payment.

Medical Wellness programs use a qualified treating NP/PA. DPC may use a qualified primary-care physician or NP/PA; registered autonomous primary-care practice is considered within its authorized scope. For aesthetics, MFMD accepts APP participation within mixed practices whose clinical activity is primarily medical; laser hair removal alone has its own acceptance pathway. Laser hair removal combined only with Botox does not require adding Medical Wellness. The assigned physician confirms procedure-specific scope according to personnel, device, and treatment area during the facility assessment.

Initial vendor and ancillary-service setup is part of the common foundation when needed. Ongoing MFMD physician participation to open and maintain pharmacy or medical-supply accounts is an optional $500/month per facility, subject to prior MFMD approval of the sources and requested support. Facilities maintaining their own accounts do not incur this monthly fee. Medicines, supplies, testing, equipment, and third-party services are separate.

Choose your services and describe your team and existing clinical documents. MFMD confirms the agreed scope, physician involvement, and activation date after review. A HIPAA-compliant digital clinical platform with documented patient encounters and physician access must be in use before services begin. Appointment software alone and paper-only records do not meet this requirement.

The initial payment is due on the effective date of the Agreement, unless a different due date is expressly stated in the payment schedule. Recurring due dates follow the agreed schedule. Governance begins on the agreed commencement date after the required clinical readiness steps are complete. Signing, payment and clinical activation are separate milestones.
