Direct Primary Care (DPC)

MFMD provides facility governance: review of clinical roles, the agreed primary-care scope, protocols, encounter documentation, results follow-up, referrals, and issues requiring escalation. Your practice’s clinicians deliver patient care.

There is no additional DPC setup charge. We confirm the clinical roster and applicable primary-care protocol as part of onboarding. Setup for other selected services is calculated separately.

Florida-registered autonomous APRNs do not require physician supervision for primary care within their authorized autonomous scope, including when they own the practice. MFMD governance is optional support for clinical quality, documentation, and compliance. Separate facility medical-director requirements, if applicable, still apply. When physician collaboration is required, the NP works under the applicable MFMD primary-care protocol.

The accepted primary-care scope may include preventive assessment, ongoing management of chronic conditions, routine medical visits, appropriate laboratory evaluation, care coordination, and referrals. The treating clinician remains responsible for patient assessment and treatment decisions.

DPC is part of Medical Wellness, at $1,500/month for that group. Selecting DPC alongside other Medical Wellness services does not add another group fee. Combining Medical Wellness with Wellness & Aesthetic is $2,500/month before applicable adjustments. One APP is included; additional APPs requiring MFMD collaboration add $500/month each. DPC has no additional setup charge. Other selected services retain their applicable setup fees.

MFMD then reviews the operating facts and documents, confirms the assigned physician, initial on-site assessment, scheduled visits, and any required treatment-day arrangements, and issues the agreed scope and start date. An EMR is required and must be in use with physician access before governance begins. You may complete a planning estimate while choosing the platform; paper-only clinical operations are not accepted. No clinical services begin merely because a form was submitted.
