There is no single “TRT clinic license” or “BHRT clinic license” in Florida. The owner defines the business structure, physical location, practitioners, services, prescribing model, medication pathway, and AHCA status before determining which requirements apply. If you offer testosterone, you are also operating a Schedule III controlled-substance service line, which adds compliance layers.
Define the legal entity, owners, physical location, services, and payer model (Sunbiz + local permitting). Then determine whether the entity requires Health Care Clinic licensure, fits a statutory exclusion, or should seek a Certificate of Exemption. Cash pay alone is not the legal answer.
Identify who evaluates, diagnoses, prescribes, administers, performs pellet procedures, monitors, and handles escalation. Because testosterone is Schedule III, prescribers need appropriate authority and current DEA registration, and the clinic needs a controlled-substance workflow.
