The purpose of the evaluation is not simply to “clear” the patient. It is to protect patient safety by determining whether the proposed treatment is appropriate for that patient, identifying contraindications or risk factors that may require modification, delay, or additional evaluation, and establishing the patient-specific treatment plan or order. Use patient-specific clinical evaluation as the controlling concept.

Is a questionnaire the same as the medical evaluation?
A questionnaire can support the evaluation, but it is not automatically the medical evaluation itself. The clinician must have enough patient-specific information to make the decision — and escalate when the information is insufficient or raises a concern.
